Registration Forms

Prior to your first session, you will need to complete registration forms. The easiest way to do this is to complete one of our online forms.  Complete either one of the Child & Adolescent Registration Form, or one of the Adult Registration Form.

If you’d prefer to use paper forms, you can complete these PDF forms, print them out and bring them with you at the time of your initial session. To open the forms below, you will need Adobe Reader or similar PDF reader software.
